HCA Healthcare heads into its October 27 quarterly print with analyst conviction building, options traders edging more cautious, and short sellers continuing to retreat.
The most notable development this week came from the analyst desk. RBC Capital raised its price target on HCA to $496 from $435 on October 7, maintaining its Outperform rating, a move that follows BMO Capital's fresh Outperform initiation at $495 two weeks earlier. Both targets sit well above the current price of $432.20. The consensus mean target of $459 implies roughly 6% upside from current levels, and the analyst recommendation differential factor scores in the 98th percentile, meaning HCA is among the most positively skewed names in the universe on broker sentiment. That bullishness is not unanimous. JPMorgan cut its target to $425 in August and holds a Neutral rating, and Morgan Stanley carries an Underweight with a $380 target. Goldman Sachs lowered its target to $485 in late July but kept its Buy rating. The broad shape of Street opinion is constructive but with a visible tail of skeptics at the lower end who point to exchange headwinds, payor mix pressure, and a $180 million EBITDA headwind from Q1 weather disruptions.
Options positioning has nudged more defensive as the earnings date approaches. The put/call ratio is running above its recent average at 0.85, roughly 1.4 standard deviations above its 20-day mean of 0.77. That is not an extreme reading, the 52-week high for the PCR is 1.33, but the direction of travel is clear: the ratio has climbed steadily since mid-September, coinciding with the run-up to the October 27 event. Short interest tells a different story. At 2.4% of the free float, it is low by any measure, and it has fallen about 9.7% over the past month. Borrow costs ticked up 30% on the week to around 0.50%, which sounds sharp but translates to an annualised cost that remains negligible for most institutional shorts. Availability is extremely loose at roughly 1,880%, meaning there are nearly 19 shares available to borrow for every one currently lent out, so there is no squeeze dynamic anywhere in the lending market.
The valuation picture is not stretched. The stock trades at about 13.8 times earnings and 9.0 times EV/EBITDA, with the EBITDA multiple compressing slightly over the past month as the price has recovered. That recovery is real: HCA is up 6.7% over the past month to $432.20, clawing back a significant portion of earlier losses. The EV/EBIT factor scores in the 79th percentile, pointing to relative value versus the broader universe. EPS momentum scores are weaker, at the 39th and 30th percentiles on the 30-day and 90-day reads respectively, reflecting the earnings revision headwinds that followed the Q2 print.
On ownership, there is a genuinely notable structural fact. Thomas F. Frist Jr. filed a Schedule 13D in February 2026 disclosing a 31.6% stake of approximately 70.6 million shares. That is an activist-designated filing, and at roughly one-third of the company it represents the dominant shareholder force in HCA's register. Frisco Holding II and Hercules Holding II, together representing related family holdings, account for a further 31.8% between them per the institutional data. That concentration means effective control sits firmly with the founding family. BlackRock and Vanguard hold 6% and 5% stakes respectively, with both marginally adding in recent months. As required disclosure: 13D/G stakes are event-driven filings around the 5% threshold, and positions are as-last-disclosed.
The last earnings release, in July, produced a one-day gain of 3.5% and a five-day move of 6.9%, the only recent data point available. With the next print three weeks away, the developing question is whether management's promised sequential improvement in cost trends through Q3 and into Q4 shows up in the EBITDA numbers, and how much of the remaining exchange headwind the market has already priced into a stock that has recovered sharply but still sits below its spring highs.
